Replacement for lost or damaged Department Card

From time to time Department Dining Cards need to be replaced. Reasons may include:

• Card was misplaced
• Card was not returned following staff turn-over
• Card is worn or damaged
• Departmental or divisional restructuring requires cards to be updated to reflect new usage

Reprinted cards will be billed to the Fund and Org provided in the Budgetary section below, at a rate of $5.44 per card.
